How to submit
Three ways: complete on paper and bring to your visit, scan and email to info@dentalexcellencetx.com, or arrive twenty minutes early and fill them in the lobby with help from the front-desk team.
For pediatric patients, a parent or legal guardian must complete the forms. Bring proof of guardianship if relevant.
